Five Whys Root Cause Analysis Training Online

Intended audience

This course is recommended for all University personnel, and especially supervisors who are responsible for investigating the root cause of employee injuries, illnesses, exposures, fires, and near misses so preventative measures can be implemented to prevent recurrence.  

Description

This course covers how to use the Five Why Technique to identify the root causes of accidents, incidents, or near misses so we can prevent recurrence. It also includes how to document incident investigations in the Online Accident Reporting System (OARS).
