This form that allows you to enter label information for four identical hazardous waste containers.

1. Fill in the label information with the chemical compounds and corresponding percentages. Be sure to select the correct hazard type on each label.
2. Print the labels Avery 5168, 3M 3100, or equivalent. Be sure to select “Actual Size” in the print options menu.
3. Affix labels to the hazardous waste containers.
