To report an accident or injury that has occurred, please submit a report through the Online Accident Reporting System (OARS).

If an accident or injury has occurred that involves a UW employee, it must be reported in OARS.

UW employees or students seeking public health guidance for a positive COVID-19 test result, close contact exposure to COVID-19 or a related issue, please contact the EH&S COVID-19 Response and Prevention Team at covidehc@uw.edu or 206.616.3344.


This reporting tool is for non-emergency incidents that occur on University of Washington owned or leased property that involve a University student, staff, faculty or other personnel, or visitor.

Please give us as much information as you are willing or comfortable providing so that we can adequately address the issue.
 
Please note that EH&S will evaluate your report and share details with relevant parties as needed. Details of the incident may be shared, however we will make every effort to protect your personal information. Please remember that all information contained within this report may become part of the University's administrative record keeping pursuant to WAC 478-140- 018.
 
Depending on the nature of the incident and the details provided, EH&S may or may not address the issue directly and we cannot guarantee a response to every reported incident.
