A person must be listed on a RAM Authorization before it is legal for that person to handle radioactive material. This process makes her or him a "certified" radiation worker.

Persons who will be in your laboratory for no more than 3 months are exempt from taking the formal Radiation Safety Training - Online and Classroom course. However, these persons may handle radionuclides only under the direct supervision of lab personnel or staff who are already certified for the use of radioactive materials by the EH&S Radiation Safety Office.

It is recommended that persons who are planning on working with radioactive materials should try to complete the training as soon as possible after joining a laboratory.

Use the Radioactive Material (RAM) Lab Personnel Change Form below if you would like to terminate or remove workers from your authorization. Only personnel changes can be made on this form.

The University of Washington is committed to providing access, equal opportunity, and reasonable accommodation for individuals with disabilities. Contact the Environmental Health & Safety Department at ehsdept@uw.edu or call (206) 543-7262 to request an accommodation.

Fields marked with an asterisk ( * ) are required.

Name of the Authorized Investigator (or SELF if you are the principal investigator, or NON if no radioactive material will be used)

Because of the training requirements of our State Radioactive Materials License, only individuals who have completed the Radiation Safety Class may be listed as radiation workers under an authorization.

Workers are added to the list of 'Authorized Reseach Participants' upon completion of the Radiation Safety Class and passing the final exam.

Only personnel changes can be made on this form i.e. lab terminations or additions, name or titl (job title) changes or moving to a different authorization.
